Leeds City Council is seeking a motivated and skilled Occupational Therapist to join our growing Rehabilitation and Recovery Service. This community-based position supports individuals transitioning from hospital to home, offering an excellent opportunity to make a genuine impact on people’s lives.
PAYE Rate: £19.20
About the Role
As part of a dedicated team of 9 Occupational Therapists, you will:
Deliver person-centred, goal-driven assessments and interventions
Promote independence and community reintegration following hospital discharge
Lead discharge planning and goal evaluation in collaboration with support workers
Complete moving & handling assessments and recommend safe, practical solutions
Collaborate closely with Physiotherapists, Nurses, Care Staff, and Social Workers
Work within a 24-hour therapeutic recovery environment in one of Leeds Recovery Hubs
Key Responsibilities
Carry out complex OT and reablement assessments
Develop and implement outcome-focused support plans
Identify and recommend specialist equipment and major adaptations
Assess housing needs and support access to suitable accommodations
Maintain accurate documentation and contribute to multidisciplinary reviews
Supervise junior and unregistered staff, and support OT students and new graduates
Participate in safeguarding, risk assessments, and major adaptation reviews
What We’re Looking For
Degree or Diploma in Occupational Therapy
HCPC Registration (Health and Care Professions Council)
Experience with strength-based approaches and moving & handling risk assessments
Skilled in working across multidisciplinary teams
Passionate about rehabilitation, inclusion, and promoting independence
