Toronto Dermatology Centre is an extremely busy clinic specializing in anti-aging and corrective treatments including neuromodulators, HA Volumizers, laser treatments and medical-grade skincare.
Job Summary:
The Nursing Patient Care Coordinator serves as a vital liaison between patients, healthcare providers, and administrative staff. The role ensures seamless care by coordinating appointments for new and existing patients and supporting patients in navigating the healthcare system. The ideal candidate will possess strong communication skills, empathy, and organizational abilities to deliver high-quality patient support.
Key Responsibilities:
• Coordinate patient care activities and schedules across various departments.
• Receive high volume of phone calls and emails and be able to prioritize and real-time patient interaction.
• Register patients and obtain all necessary demographic information and accurately input into electronic medical records (EMR) system
• Communicate effectively with patients regarding follow-ups, referrals and prescription renewal requests.
• Assist in managing pre-authorization and insurance forms for patients.
• Educate patients about minor procedures, and follow-up requirements.
• Act as a point of contact for patient questions or concerns, resolving issues promptly or directing them to appropriate personnel.
• Support providers by ensuring efficient workflow and proper documentation.
• Collaborate with interdisciplinary teams to promote patient -centered care.
Essential for success in this role:
• Attention to detail is critical.
• Must be able to work well within a team environment.
• Must be able to use professional communication over phone, written word, and in person.
• Very strong computer proficiency and the ability to multi-task.
• Knowledge of the EMR system an asset.
• Complete fluency of the English language, including excellent spelling and grammar.

Pay: $21.00-$23.00 per hour
